Health and Dental Plans
Self-funded health and dental plans allow an employer to work with an experienced agent to formulate a tailor-made plan that includes the benefits you wish to provide. The agent will equip you with a standard plan design that complies with all requirements of current laws. The employer can then choose the desired benefits and payment levels.

Our staff of claims examiners processes claims through a software that is programmed with your plan design. Examiners are assigned specific employer groups and handle all aspects of claims processing for those groups. The examiners utilize their experience and working knowledge to properly adjudicate claims and answer questions from both insureds and providers.

Short Term Disability
Short term disability payments will be calculated and issued by Custom Benefit Administrators on an employer’s behalf after receiving proper documentation of a disability. Payment periods will be monitored.

COBRA - Consolidated Omnibus Budget Reconciliation Act
HIPAA - Health Insurance Portability and Accountability Act

Frequent and ongoing legislation has created many changes to both of these laws since their original enactment. CBA can handle numerous requirements of these two federal laws which govern your health and/or dental plan and assist employers with all administrative guidelines. Our staff members participate in ongoing training in order to maintain full compliance. We provide initial notification of rights and responsibilities associated with the laws. COBRA election packets are generated and timely response and payments are closely monitored.

If an employer does wish to administer their own COBRA and HIPAA, we can provide the necessary, fully compliant forms.

Section 125 Cafeteria Plans
Section 125 Cafeteria plans offer payment of non-covered medical and dental related services using pre-tax dollars. The plan can also be used for day care expenses and insurance premium payments. Custom Benefit Administrators will process reimbursement requests from employees and monitor account balances.
